Terms and Conditions

I give my consent for the Lactation Consultant IBCLC Bethany Daniell to work with me and my baby during this consultation for my breastfeeding problem/concern. This consent is for visits, phone conversations, and information sent by e-mail, or text, and includes appropriate follow-up contacts.

I understand that a lactation consultation may involve:

• touching my breasts and/or nipples for the purposes of assessment;

• inserting gloved fingers into my baby’s mouth to assess suck or oral restriction;

• observation of a breastfeed, and suggestions to optimise latch or position.

• demonstration of equipment or supplies and techniques.

• The Lactation Consultant will ask prior to touching myself or my babies. My consent for physical touch can be withdrawn and will be respected at any time during the consult.

• The Consult fee covers the initial home visit up to 1.5 hours and one follow-up phone call / text / email. Additional consultation will incur additional fees. Additional fees will be discussed prior to provision of any additional services. 

•  Consumables such as infant feeding equipment or specialty devices are not included in consult fee and will incur additional charges.

• I understand that any cancellation of appointments without 24 hours notice will incur a cancellation fee of 50% of my consultant fee. If I fail to attend any appointments, a cancellation fee of 100% of my consultant fee is charged. This is due to the lactation consultant being unable to offer this appointment to another client without sufficient notice.

• I give my consent for the lactation consultant to use deidentified clinical information obtained during our sessions for education of other health care providers and mothers about lactation. I won’t be identified in any way, but aspects of my situation may be described and discussed.

• I understand total payment is expected at the time of booking the consultation and any default of payment will be forwarded to a collection agency.
